The road to mental health can be bumpy. That’s why your Care Partner supports you every step of the way.

What is a Care Partner?

As soon as you join, you are assigned a dedicated Care Partner, who acts as your primary touchpoint throughout your care journey. Prairie’s Care Partners are trained mental health professionals with years of experience providing mental healthcare.

Your Care Partner supports you over text and monthly video calls in 5 key ways:

Coaching

Care Partners track and work with you to meet your mental health goals. They also reach out from time to time to see how you’re doing and make sure you’re okay.

Counseling

Care Partners share mental health resources, use evidence-based techniques like cognitive behavioral therapy (CBT), and support you over text during bad days.

Note: Care Partners are not licensed therapists and do not provide care meant to replace therapy.

Medication Support

Once prescribed, Care Partners can answer basic questions regarding your prescription or consult with your psychiatrist in between your visits.

Care Coordination

Care Partners can coordinate care with your other providers (like PCPs, OB-GYNs and therapists) so you can get the best care possible, even if it’s not with us.

Administrative support

Care Partners can help you with any questions or issues that arise, from billing to completing your genetic test to getting a referral to a therapist.

Your Care Partner makes sure you never feel alone

Daily messaging with your Care Partner means someone is there for you every step of the way. Our Care Partners proactively check in with you every week answer your questions.
